Download our new patient forms and fill them out anytime before you visit. E-mail them to us or print them out and bring them with you.

As you walk in you’ll meet our friendly front staff, who will walk you through the new patient paperwork & verify your insurance benefits for you (if applicable). You will then take photos for Dr. Chung to review during the consultation.

You’ll then meet with Dr. Chung for your complimentary orthodontic consultation. She will review your photos with you and complete a comprehensive clinical exam. Finally, she'll review your final diagnosis & recommended treatment plan.

If after the consultation, it is determined that treatment is indicated – you may begin your diagnostic records that day. These records take about 30 minutes, and usually consist of study models (3D Digital Scan is taken with our iTero Intraoral Scanner®), facial & dental photographs and two orthodontic x-rays (if indicated).
